Bronchoscopic foreign body removal from airway- Removal of foreign body from tracheobronchial tree is very challenging but very gratifying in nature. Accidental inhalation of foreign bodies are not very uncommon but it is particularly frequent in younger age group. presentation of FB aspiration may vary widely and range from impending asphyxia to seemingly stable condition. symptoms would depend upon size of FB, location of FB, nature of FB and mechanism of obstruction (simple obstruction vs ball valve mechanism). Patient mentioned in this news paper is known patient of post TB COPD with Lt destroyed lung. She aspirated a peanut while having breakfast. As patient already had compromised lung function, following aspiration her condition worsened rapidly and she developed acute respiratory failure. In view of respiratory failure and high work of breathing, patient had to be put on mechanical ventilation. Inspection bronchoscopy was done which revealed a peanut impacted in RT intermediate bronchus. Subsequently peanut was dislodged using dormia basket and pulled in to endo tracheal tube which was removed in toto. following removal of foreign body, patient's respiratory failure improved rapidly and patient was weaned from ventilator after 2 hour. Patient was discharged from hospital by evening. Here timely removal of this organic foreign body has most definitely saved the life of this patient.
